Appointments

Appointments can be scheduled for the following times:

• Monday – Friday 9:00 am – 5:00 p m
• After hours and Saturdays by appointment only

Doctors must complete the prescription form available on our website. The form can be faxed, e-mailed, or given to patients to bring to their appointment.
